This Notice explains how the Hospice Association of Witwatersrand (hereinafter referred to as HospiceWits and the organisation) obtain, use and disclose your personal information, as is required by the Protection of Personal Information Act (“POPI”).
At HospiceWits, we are committed to protecting your privacy and to ensure that your personal information is collected and used properly, lawfully and transparently.

- Who we are
In this Notice, HospiceWits is a non-profit organisation operating within the borders of South Africa.
- The information we collect
HospiceWits collects information for a number of purposes, e.g the personal details of HospiceWits patients’ and their next-of-kin to capture on the patient records for the In-Patient Unit (IPU). An indemnity is signed when this information is collected, agreeing to the terms for which the information will be used.
HospiceWits collects information of our donors. Where possible, the organisation will inform you what information you are required to provide to us and what information is optional.
HospiceWits also collects information of its students for the training courses that the organisation provides. This information is used for registration, issuing of certificates and record keeping. The students sign an indemnity form upon registering, agreeing to the terms for which the information will be used.
- How we use your information
HospiceWits will use your personal information only for the purposes for which it was collected or agreed with you, for example:
- To provide our services to you, to carry out the services you requested and to maintain our relationship;
•To confirm and verify your identity;
• For the detection and prevention of fraud, crime or other malpractice;
• For audit and record keeping purposes;
• In connection with legal proceedings.
HospiceWits will also use your personal information to comply with legal and regulatory requirements or industry codes to which the organisation subscribes or which apply to us, or when it is otherwise allowed by law.
- Disclosure of Information
HospiceWits may disclose your personal information to the organisation’s service providers who are involved in the delivery of services to you. HospiceWits has policies in place to ensure that they comply with these privacy terms.
HospiceWits may also disclose your information:
- Where the organisation has a duty or a right to disclose in terms of law or industry codes;
•Where the organisation believe it is necessary to protect our rights.
- Information Security
HospiceWits is legally obliged to provide adequate protection for the personal information the organisation holds and to stop unauthorised access and use of personal information. HospiceWits will, on an ongoing basis, continue to review our security controls and related processes to ensure that your personal information is secure.
HospiceWits’ security policies and procedures cover:
- Physical security;
•Computer and network security;
• Access to personal information;
• Secure communications;
• Security in outsourced contractors;
• Retention and disposal of information;
• Acceptable usage of personal information;
• Governance and regulatory issues;
• Monitoring access and usage of private information;
• Investigating and reacting to security incidents.
In the event HospiceWits contracts with third parties, the organisation imposes appropriate security, privacy and confidentiality obligations on them to ensure that personal information that we remain responsible for, is kept secure. HospiceWits will ensure that anyone, to whom the organisation passes your personal information onto, agrees to treat your information with the same level of protection, as the organisation is obliged to.
- Your Rights: Access to information
You have the right to request a copy of the personal information HospiceWits holds about you. To do this, you may contact the organisation and you will be directed to the relevant department. HospiceWits will take all reasonable steps to confirm your identity before providing details of your personal information.
- Correction of your information
You have the right to ask us to update, correct or delete your personal information. You may do this by contacting the organisation and you will be directed to the relevant department. HospiceWits will take all reasonable steps to confirm your identity before making changes to personal information we may hold about you.
- Changes to this Notice
Please note that HospiceWits may amend this Notice from time to time. Please check this website periodically to inform yourself of any changes.
